Employees usually need one for a site induction, a workplace requirement, or their own records. In Australia there are three ways to get one: the certificate your vaccination provider issues (every employee vaccinated through a Corporate Care workplace program receives one), your official immunisation history statement through myGov, or a letter from the pharmacist or doctor who gave you the vaccine.
Because providers must report every flu vaccination to the Australian Immunisation Register (AIR), your vaccination also appears on your official immunisation history statement. Lose the certificate and the record is still there.
How to get your flu vaccine certificate
Which path you take depends on where you were vaccinated. All three produce proof an employer can accept.

Your Corporate Care certificate
Every employee vaccinated through a Corporate Care onsite clinic who books online receives a PDF vaccination certificate. It records your name, vaccination date, vaccine brand and batch, and the Registered Nurse who administered it. Need it re-sent? Email flu@corporatecare.com.au and we’ll reissue it.

Your immunisation history statement
Your official vaccination record, drawn from the Australian Immunisation Register. Sign in to myGov, open your linked Medicare service, and download the statement as a PDF — any time, no charge.

A letter from whoever vaccinated you
Vaccinated at a pharmacy or by your GP? Ask them for written confirmation. Any vaccination provider can issue a statement or letter confirming the vaccine they gave you, and the vaccination will also be on your AIR record.
Flu vaccine certificate sample: what yours shows
If you’ve never seen one, this is what to expect. A certificate is a one-page document that identifies you, the vaccine, and the person who administered it. That’s all anyone checking it needs to verify.
A flu vaccination certificate typically shows:
- Your full name and date of birth
- The vaccination date and where it was given
- The vaccine — brand and batch number
- Who administered it — for our programs, an AHPRA-registered Registered Nurse
What counts as proof of flu vaccination in Australia
People search for this under different names — flu vaccine certificate, immunisation record, vaccination statement — and land in different places. If someone has asked you for “proof of flu vaccination”, any of these does the job:
- A provider certificate — like the Corporate Care certificate above
- Your immunisation history statement — the official record from myGov. It comes in two forms: the complete statement, or a COVID-19 and influenza (flu) statement that shows just those vaccinations
- A letter or record card from the pharmacist or doctor who vaccinated you
The statement is the strongest general-purpose answer because it’s the official register record, it’s free, and you can download it again any time. The certificate is the fastest: it’s already in your inbox. If the organisation asking specifies a format, use theirs; if not, either works.
Certificates in our workplace programs

Your booking, consent, and vaccination record live in one system, your certificate is generated from that record, and the same record is reported to the Australian Immunisation Register — so your myGov statement backs up your certificate.
Lost it two months later, on the morning of an aged-care site visit? Email us and we’ll reissue it from the program record. That’s the practical difference a system makes: the certificate is a document, but the proof is the record behind it.

Frequently Asked Questions
If you were vaccinated through a Corporate Care workplace program, email flu@corporatecare.com.au with your name and your employer's name, and we'll reissue your certificate from the program record.
You can also download your immunisation history statement from myGov at any time — your flu vaccination is on your Australian Immunisation Register record, and that statement is accepted as official proof.
Employees who book online receive their PDF certificate as part of the program.
Your vaccination also appears on your immunisation history statement in myGov once it's processed by the Australian Immunisation Register.
There is no general legal requirement to hold a flu vaccination certificate. Some workplaces, client sites, and sectors ask for proof of vaccination as part of their own policies — common examples are site inductions and roles that involve visiting health or aged-care facilities.
If you're asked for proof, your provider certificate or your immunisation history statement both do the job.
A certificate is issued by the provider who vaccinated you — for our programs, that's Corporate Care — and covers that vaccination.
An immunisation history statement is the official record of everything on your Australian Immunisation Register record, downloaded from myGov. Because providers must report flu vaccinations to the register, a vaccination on your certificate should also appear on your statement.
